Summary Care record
If you are registered with a GP practice in England, you will have a Summary Care Record (SCR), unless you have previously chosen not to have one. It includes important information about your health:
- Medicines you take
- Allergies you have
- Any bad reactions to medicines.
Essential details about your healthcare can be difficult to remember, particularly when you are unwell, so having an SCR means staff involved in your care can make better and safer decisions.
Certain vulnerable patient groups such as those with dementia or with detailed and complex health problems can particularly benefit from additional information in their SCR.
Additional information can be added to your SCR by your GP practice at your request.
Such as your :
- long term health conditions e.g. asthma, diabetes or heart problems .
- healthcare needs & personal preferences –you may have a communication need, a long term condition that needs to be managed in a particular way, or have preferences about your care.
- vaccinations or operations you have had in the past
